New Carrier Checklist
- MC Authority Number (if applicable)
If you have a Motor Carrier Authority, please provide your MC number.
- DOT Number
Include your Department of Transportation (DOT) number and any relevant state permits.
- Proof of Insurance
Please provide proof of both liability and cargo insurance coverage.
- Completed W-9 Form
Submit a completed W-9 form for tax purposes.
